Indications/Uses
Management of renal tubular acidosis w/ Ca stones; hypocitraturic Ca oxalate nephrolithiasis of any etiology; uric acid lithiasis w/ or w/o Ca stones. Prevention of stone relapse after extracorporeal shock wave lithotripsy or lithotomy.
Dosage/Direction for Use
Severe hypocitraturia (urinary citrate <150 mg/day) 60 mEq/day in 2 or 3 divided doses (20 mEq tid or 30 mEq bid). Mild to moderate hypocitraturia (urinary citrate >150 mg/day) 30 mEq/day in 2 or 3 divided doses (10 mEq tid or 15 mEq bid).
Administration
Should be taken with food: Take tab 30 min after meals. Swallow whole, do not crush/chew/suck. Avoid food w/ high salt content. Increase fluid intake.
Contraindications
Hypersensitivity. Patients w/ hyperkalemia or conditions predisposing them to hyperkalemia eg, chronic renal failure, uncontrolled DM, familial hyperkalemic periodic paralysis (adynamia episodica hereditaria), acute dehydration, strenuous physical exercise in unconditioned individuals, adrenal insufficiency, extensive tissue breakdown, severe myocardial damage, administration of K-sparing diuretic (eg, triamterene, spironolactone or amiloride); slow gastric emptying, esophageal compression, intestinal obstruction or stricture; active PUD; active UTI (w/ either urea-splitting bacteria or other organisms, in association w/ either Ca or struvite stones); renal insufficiency (GFR <0.7 mL/kg/min); resp or metabolic acidosis. Patients on anticholinergic therapy.
Special Precautions
Discontinue treatment in the occurrence of hyperkalemia, a significant rise in serum creatinine, or a significant fall in blood hematocrit or Hb. Immediately discontinue treatment & investigate for possibility of bowel perforation or obstruction in the event of severe vomiting, abdominal pain or GI bleeding. Regular serum K monitoring is recommended. Periodically determine acid-base balance, other serum electrolyte levels, & ECG. Careful attention should be given to patient's clinical status, particularly in the presence of cardiac disease, renal disease or acidosis. Can produce hyperkalemia & cardiac arrest in patients w/ impaired mechanisms for K excretion. Give to pregnant woman only if clearly needed. Caution in breastfeeding woman. Safety & efficacy in childn have not been established.
Adverse Reactions
Minor GI complaints eg, abdominal discomfort, vomiting, diarrhea, loose bowel movements & nausea.
Drug Interactions
Severe hyperkalemia w/ K-sparing diuretics (eg, triamterene, spironolactone, or amiloride). Increased GI irritation caused by K salts w/ drugs that slow GI transit time (eg, anticholinergics).
