Contents
Per Metromic supp Metronidazole 500 mg, miconazole nitrate 100 mg. Per Metromic Forte supp Metronidazole 750 mg, miconazole nitrate 200 mg
Indications/Uses
Candidal vulvovaginitis due to Candida albicans, in bacterial vaginosis due to anaerobic bacteria & Gardnerella vaginalis, in trichomonal vaginitis due to Trichomonas vaginalis & in mixed vag infections.
Dosage/Direction for Use
Metromic Insert 1 supp high into the vagina at night & in the morning for 7 days. Recurrent case 1 supp in the morning & 1 supp at night for 14 days. Metromic Forte Insert 1 supp high into the vagina at night for 7 days. Recurrent case 1 supp at night for 14 days.
Contraindications
Hypersensitivity. Porphyria, epilepsy, serious liver function disorders. Concomitant use w/ alcohol (during or w/in 3 days after treatment); disulfiram (during treatment or w/in 2 wk). 1st trimester of pregnancy.
Special Precautions
Not to be swallowed or applied by other routes. High doses & long term systemic use may cause peripheral neuropathy & convulsion. Do not take alcohol during & for 3 days after end of treatment. Concurrent use w/ rubber or latex condoms or diaphragms is not recommended. Do not use other vag products (eg, tampon, douche or spermicide) during treatment. Not be used in young females who are not sexually active; girls who are not sexually mature. Treat sexual partners w/ Trichomonas vaginalis at the same time. Do not drive or operate machinery if dizziness, ataxia, fatigue & weakness occur. Renal insufficiency. Severe hepatic insufficiency; hepatic encephalopathy. Women of childbearing potential should avoid pregnancy w/ proper birth control method. Not to be used unless necessary in 2nd & 3rd trimesters of pregnancy. Discontinue breastfeeding (can be started again 24-48 hr after end of treatment). Elderly >65 yr.
Adverse Reactions
Vag discharge. Dizziness, headache; vaginitis, vulvovag irritation, pelvic discomfort.
Drug Interactions
Inhibited metabolism & increased plasma conc of astemizole & terfenadine. Increased toxicity of cyclosporine. Metronidazole: Disulfiram-like reaction w/ alcohol. Increased risk of cardiotoxicity w/ amiodarone. Increased blood conc of carbamazepine. Increased blood level & risk of neurologic side effects w/ cimetidine. CNS-related effects w/ disulfiram. Increased blood levels & toxicity of fluorouracil. Increased toxicity of lithium. Increased anticoagulant effect of oral anticoagulants. Increased blood levels of phenytoin. Decreased blood levels w/ phenytoin; phenobarb. Interference w/ blood levels of liver enzymes, glucose (hexokinase method), theophylline & procainamide. Miconazole nitrate: Increased bleeding risk w/ acenocoumarol, anisindione, dicumarol, phenindione, phenprocoumon, warfarin. Inhibited metabolism & increased plasma conc of cisapride. Reduced metabolism of carbamazepine. Increased or prolonged effects of opioid. Increased hypoglycemic action of glimepiride. Increased plasma conc or exposure to oxybutynin. Increased plasma conc & reduced clearance of oxycodone. Increased risk of toxicity of phenytoin; trimetrexate. Increased risk of cardiotoxicity w/ pimozide.
