Metoclopramide HCl

Indications/Uses

Disorders of decreased GI motility eg, gastroparesis or ileus; in GERD & dyspepsia; & in nausea & vomiting associated w/ various GI disorders, w/ migraine, following surgery, & w/ cancer therapy. Used to stimulate gastric emptying during radiographic exam, to facilitate intubation of small bowel, & in management of aspiration syndromes.

Dosage/Direction for Use

IM/IV Over 1-2 min. Nausea & vomiting Adult 10 mg tid. Childn 15-19 yr (<60 kg) 5 mg tid, 9-15 yr (≥30 kg) 5 mg tid, 5-9 yr (20-29 kg) 2.5 mg tid, 3-5 yr (15-19 kg) 2 mg tid, 1-3 yr (10-14 kg) 1 mg bid-tid, up to 1 yr (up to 10 kg) 100 mcg/kg (Max: 1 mg) bid. Diagnostic procedures As a single dose, 5-10 min before exam. Adult 10-20 mg. Childn 15-19 yr 10 mg, 9-14 yr 5 mg, 5-9 yr 2.5 mg, 3-5 yr 2 mg, <3 yr 1 mg.

Contraindications

Hypersensitivity (patients sensitive to procaine & procainamide may be sensitive to metoclopramide). Pheochromocytomas. Patients in whom increased GI motility might be dangerous eg, presence of GI haemorrhage, mechanical obstruction or perforation. Porphyria. Not to be used in patients w/ epilepsy. Not to be administered to patients receiving other drugs which are likely to cause extrapyramidal reactions.

Special Precautions

Not to be used when stimulation of muscular contractions might adversely affect GI conditions as in GI haemorrhage, obstruction, perforation, or immediately after surgery. Hypertensive crises in patients w/ phaeochromocytoma. Review patients on prolonged therapy regularly. Patients w/ renal impairment, epilepsy, Parkinson's disease, or history of depression. May cause drowsiness or impaired reactions; patients affected should not drive or operate machinery. Childn, young patients, & elderly; patients <20 yr.

Adverse Reactions

Extrapyramidal symptoms which usually occur as acute dystonic reactions commonly in childn & young adults, especially if female, & at daily doses >500 mcg/kg. Parkinsonism & tardive dyskinesia usually during prolonged treatment in elderly. Restlessness, drowsiness, & diarrhea. Hypotension, HTN, dizziness, headache, & depression. Disorders of cardiac conduction w/ IV metoclopramide. Galactorrhea or related disorders. Transient increases in plasma-aldosterone conc.

Drug Interactions

Other drugs that can also cause extrapyramidal reactions eg, phenothiazines. Increased toxicity w/ lithium, & other centrally active drugs eg, antiepileptics. Antagonized GI effects w/ antimicrobics & opioid analgesics. May affect absorption of other drugs; may either diminish absorption from stomach (as w/ digoxin) or enhance absorption from small intestine (as w/ ciclosporin or paracetamol). May prolong neuromuscular blockade produced by suxamethonium & mivacurium. May increase prolactin blood conc & therefore interfere w/ drugs which have hypoprolactinaemic effect eg, bromocriptine. Not to be given to patients receiving MAOIs.
