Levofloxacin hemihydrate

Indications/Uses

Wide range of infections including lower resp tract & skin infections, typhoid & paratyphoid fever, & UTI. Prophylaxis of meningococcal meningitis & surgical infections.

Dosage/Direction for Use

Adult 250-500 mg once daily or bid for 7-14 days.

Overdosage

View Levolox overdosage for action to be taken in the event of an overdose.

Administration

May be taken with or without food: Take at least 2 hr before or 2 hr after antacids containing Ca/Mg/Al, after sucralfate, didanosine or metal cations (eg, Fe), multivit prep w/ Zn.

Contraindications

Not to be used during pregnancy or lactation & in childn & adolescents.

Special Precautions

Discontinue use if patients experience tendon pain, inflammation or rupture; report side effects suggestive of aortic aneurysm or dissection. Avoid use in methicillin-resistant Staph aureus infection. Not be used in patients at increased risk including those w/ history of blockages or aneurysm of the aorta or other blood vessels, high BP, certain genetic disorders involving blood vessels (ie, Marfan & Ehlers-Danlos syndrome), & elderly. Patients w/ epilepsy or history of CNS disorders; G6PD deficiency or myasthenia gravis. Maintain adequate fluid intake during treatment. Avoid exposure to strong sunlight. May impair ability to drive or operate machinery especially w/ alcohol. Hepatic or renal impairment.

Adverse Reactions

Nausea, vomiting, diarrhea, abdominal pain, dyspepsia; headache, dizziness, restlessness.

Drug Interactions

GI absorption may be interfered w/ antacids containing Ca, Mg, Al; sucralfate, didanosine, metal cations (eg, Fe) & multivit prep w/ Zn. Potential additive effect on QT interval prolongation w/ class Ia (eg, quinidine, procainamide) or III (eg, amiodarone, sotalol) antiarrhythmics; fluoxetine or imipramine. Blood glucose disturbances including hyperglycemia & hypoglycemia w/ antidiabetics. Possible increased AUC of ciclosporin or tacrolimus. Risk of tendon rupture may be increased w/ corticosteroids particularly in the elderly. Episode of torsades de pointes w/ fluconazole in patient on hemodialysis. May increase risk of CNS stimulation & convulsive seizures w/ NSAIDs. Potential increased AUC & t_1/2 w/ probenecid & cimetidine. Prolonged elimination t_1/2 & elevated serum levels of theophylline. Enhanced effects of warfarin.
