Dosage/Direction for Use
Adult and Adolescent: Intramuscular or Intravenous Infusion: 750 mg to 1.5 grams (base) every 8 hours.
Note: Gonococcal Infections (Uncomplicated): Intramuscular: 1.5 grams (base), divided into 2 doses and administered at two separate sites and 1 gram of probenecid orally, administered simultaneously as single dose.
Meningitis, Bacteria: Intravenous: up to 3 grams (base) every eight hours.
Preoperative Prophylaxis in Open Heart Surgical Patients: Intravenous: 1.5 grams (base) ½ to 1 hour prior to the start of the surgery; 750 mg intravenously or intramuscularly every 8 hours if the surgical procedure is prolonged for up to 24 hour.
Pediatric Dose: Neonates: Intramuscular or Intravenous: 10 to 33.3 (base) per kg of body weight every 8 hours; or 15 to 50 mg per kg of body weight every 12 hours.
Infants and Children 3 Months of Age and Over: Intramuscular or Intravenous: 16.7 to 33.3 mg (base) per kg of body weight every 8 hours.
Note: Bone Infections: Intravenous: 50 mg (base) per kg of body weight every 8 hours meningitis.
Bacteria (Neonates): Intravenous: 33.3 mg (base) per kg of body weight 8 hours or 50 mg per kg of body weight every 12 hours.
Pediatric patients with impaired renal function may require a reduction in the frequency of dosage consistent with adult dosage recommendations in renal impairment.
