Indications/Uses
Resp tract infections, mild to moderate skin & soft tissue infections, otitis media, & H. pylori eradication.
Dosage/Direction for Use
FC tab Adult 250 mg bid, may be increased to 500 mg bid in severe infection. Childn 15 mg/kg daily in 2 divided doses. Max: 500 mg every 12 hr. Duration: 5-14 days (6-14 days for community-acquired pneumonia & sinusitis). Granules for susp Childn 15 mg/kg daily in 2 divided doses. Max: 500 mg every 12 hr. Duration: 5-14 days (6-14 days for community-acquired pneumonia & sinusitis). Childn 7-12 yr 7.5-10 mL every 12 hr, 2-6 yr 5 mL every 12 hr, 6 mth-1 yr 2.5 mL every 12 hr.
Administration
May be taken with or without food: Granules for susp: Add approx 40 mL water to make 50 mL reconstituted susp. Shake well until granules are evenly suspended.
Contraindications
Hypersensitivity to clarithromycin, erythromycin, or any macrolides. Granules for susp: Patients w/ history of cholestatic jaundice/hepatic dysfunction associated w/ prior use of clarithromycin. Concomitant administration w/ cisapride, pimozide, astemizole, terfenadine, & ergotamine or dihydroergotamine. Co-administration w/ colchicine in patients w/ renal or hepatic impairment.
Special Precautions
Patients w/ renal or hepatic impairment. Reduce dose in patients w/ severe renal impairment. Pregnancy. Granules for susp: Discontinue use in case of signs & symptoms of hepatitis; suspected or confirmed Clostridium difficile-associated diarrhea. Safety has not been established during breast-feeding of infants.
Adverse Reactions
GI disturbances; smell & taste disturbances, stomatitis, glossitis, tongue & tooth discoloration; transient elevations of liver enzymes value, cholestatic jaundice, & hepatitis; headache & rashes; transient CNS effects eg, anxiety, dizziness, insomnia, hallucinations, & confusion; hearing loss.
Drug Interactions
May inhibit metabolism & elevate serum levels of drugs metabolized by CYP3A isoenzyme eg, alprazolam, astemizole, carbamazepine, cilostazol, cisapride, cyclosporine, disopyramide, ergot alkaloids, lovastatin, methylprednisolone, midazolam, omeprazole, oral anticoagulants (eg, warfarin), pimozide, quinidine, rifabutin, sildenafil, simvastatin, tacrolimus, terfenadine, triazolam, & vinblastine; other CYP450 isoenzymes eg, phenytoin, theophylline, & valproate.
