Indications/Uses
Reduction of atherosclerotic events (MI, stroke or vascular death) in patients w/ atherosclerosis documented by recent stroke, MI, or established peripheral arterial disease. Patients suffering from non-ST segment elevation, acute coronary syndrome (unstable angina or non-Q wave MI) including patient undergoing stent placement following percutaneous coronary intervention or combination w/ ASA in medically treated patients eligible from thrombolytic therapy.
Dosage/Direction for Use
Acute coronary syndrome 300 mg loading dose, followed by 75 mg daily. Patient w/ non-ST segment elevation acute coronary syndrome Initially 300 mg loading dose & continued to 75 mg daily w/ ASA of not more than 100 mg to avoid the risk of bleeding. Patient w/ ST-segment elevation acute MI 75 mg single daily dose w/ loading dose of 300 mg in combination w/ ASA w/ or w/o thrombolytics. Elderly 75 yr Initially 75 mg. Patient w/ atrial fibrillation 75 mg single dose daily in combination w/ ASA of doses 75-100 mg daily. Prophylaxis of thromboembolic events 75 mg once daily. Management of acute coronary syndromes, unstable angina & non-Q wave MI 300 mg loading dose, followed by 75 mg once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Active pathological bleeding eg, peptic ulcer or intracranial hemorrhage.
Special Precautions
Hematopoietic disorders eg, neutropenia or thrombocytopenia, hemorrhagic diathesis or other hemorrhagic disorders associated by a prolonged bleeding time or conditions w/ an increased risk of bleeding eg, GI ulcers, acute cerebral hemorrhage or severe liver dysfunction. Perform full blood counts before starting treatment & every 2 wk during 1st 3 mth of therapy. Discontinue therapy 10-14 days before elective surgery. Thrombotic thrombocytopenic purpura may occur after a short exposure (<2 wk). Discontinue when manifestation of acquired hemophilia occurs.
Adverse Reactions
GI disturbances, skin rashes, blood dyscrasias including neutropenia & thrombocytopenic purpura, hemorrhagic disorders, hepatitis & cholestatic jaundice.
Drug Interactions
Potentiates the effect of aspirin on collagen-induced platelet aggregation. Risk of increased occult GI blood loss w/ NSAIDs. May interfere w/ the metabolism of phenytoin, tamoxifen, tolbutamide, warfarin, torsemide, fluvastatin.
