Indications and Dosage
Topical/Cutaneous
Scalp psoriasis
Adult: As 0.005% scalp solution: Apply to affected area(s) of the scalp bid. Max: 60 mL of scalp solution (equal to 3 mg of calcipotriol) per week. If used concomitantly with other products containing calcipotriol, the total dose of calcipotriol must not exceed 5 mg weekly. Available products and recommendations may vary between countries (refer to local or specific product guidelines).

Topical/Cutaneous
Plaque psoriasis
Adult: As 0.005% cream or ointment: Apply a thin film to affected area(s) 1-2 times daily. Max: 100 g of cream or ointment (equal to 5 mg of calcipotriol) per week. If used concomitantly with other products containing calcipotriol, the total dose of calcipotriol must not exceed 5 mg weekly. Available products and recommendations may vary between countries (refer to local or specific product guidelines).
Child: As 0.005% ointment: 6-12 years Apply a thin film to affected area(s) bid. Max: 50 g of ointment per week; >12 years Apply a thin film to affected area(s) bid. Max: 75 g of ointment per week. Renal Impairment
Severe: Contraindicated.
Hepatic Impairment
Severe: Contraindicated.
Contraindications
Hypercalcaemia, Ca metabolism disorders, evidence of vitamin D toxicity; acute psoriatic eruption (scalp solution). Severe renal and hepatic impairment. Contraindications may vary between individual products (refer to specific product labelling for detailed information).
Special Precautions
Not indicated for the treatment of guttate, erythrodermic, exfoliative, and pustular psoriasis. Concurrent use with vitamin D products, Ca supplements, or other agents that increase serum Ca levels. Children and elderly. Pregnancy and lactation.
Adverse Reactions
Significant: Hypercalcaemia, contact dermatitis (including allergic contact dermatitis); irritation of lesions or surrounding uninvolved skin.
General disorders and administration site conditions: Application site pain.
Immune system disorders: Hypersensitivity reactions.
Skin and subcutaneous tissue disorders: Pruritus, skin rash, burning sensation or stinging of skin, erythema, exacerbation of psoriasis, xeroderma, skin exfoliation, bullous reactions, eczema, skin oedema, photosensitivity reaction.
Patient Counseling Information
Limit or avoid excessive exposure to natural or artificial sunlight, or phototherapy. Avoid application to the face or eyes, and carefully use on skin folds.
Monitoring Parameters
Monitor serum Ca. Assess for systemic reactions or adverse skin reactions (particularly when used to large areas of the skin or for extensive periods).
Overdosage
Symptoms: Hypercalcaemia, which is exhibited by muscle weakness, constipation, polyuria, confusion and coma. Management: Discontinue treatment and monitor the serum Ca weekly until levels are normalised.
Drug Interactions
Increased risk of significant hypercalcaemia when concomitantly used with systemic vitamin D products, Ca supplements, and other agents that may increase serum Ca levels (e.g. thiazide diuretics, estrogen, anabolic steroids, parathyroid hormone, parathyroid hormone analogues).
Action
Description:
Mechanism of Action: Calcipotriol, an antipsoriatic agent, is a synthetic vitamin D3 derivative that helps regulate skin cell production and proliferation. It binds to vitamin D receptors, which induce differentiation and inhibit proliferation of keratinocytes, thereby reversing changes in abnormal keratinocytes, preventing epidermal growth, and restoring normal structure to the skin.
Synonym(s): Calcipotriene.
Onset: After 2 weeks (improvement of psoriasis).
Pharmacokinetics:
Absorption: Slightly absorbed systemically.
Metabolism: Metabolised into inactive metabolites.

Storage
Cream or ointment: Store between 15-25°C. Scalp solution: Store between 20-25°C. Keep the scalp solution away from open flame and protect it from light. Do not refrigerate or freeze the cream, ointment, or the scalp solution.
