Indications/Uses

Reversal of rocuronium- or vecuronium-induced neuromuscular blockade in patients ≥2 yr.

Dosage/Direction for Use

Administered IV as single bolus inj. Adult Routine reversal 4 mg/kg if recovery has reached at least 1-2 post-tetanic counts (PTC) or 2 mg/kg if spontaneous recovery has occurred up to at least the reappearance of T₂ following rocuronium- or vecuronium-induced blockade. Immediate reversal of rocuronium-induced blockade 16 mg/kg administered 3 min after a bolus dose of rocuronium Br 1.2 mg/kg. Re-administration Repeat dose of 4 mg/kg after an initial dose of 2 mg/kg or 4 mg/kg in exceptional situation of neuromuscular blockade post-op. Childn & adolescent 2-17 yr Routine reversal 4 mg/kg if recovery has reached at least 1-2 PTC or 2 mg/kg at reappearance of T₂.

Overdosage

Contraindications

Hypersensitivity. Patients w/ severe renal impairment, including patients requiring dialysis (CrCl <30 mL/min).

Special Precautions

Monitor resp function during recovery/coagulation parameters in patients w/ known coagulopathies or taking anticoagulants. Recurrence of neuromuscular blockade. If rocuronium re-administration is required, 24-hr waiting time is recommended. Severe hepatic impairment. Occurrence of light anesth, in rare instances marked bradycardia; use in ICU. Not to be used for reversal of block induced by nonsteroidal neuromuscular blockers (eg, succinylcholine or benzylisoquinolinium compd) or steroidal neuromuscular blockers other than rocuronium. Displacement interactions w/ toremifene & fusidic acid. Longer recovery time in CV disease, old age or edematous state. Possibility of drug hypersensitivity reactions. Patients on controlled Na diet. Pregnancy & lactation. Childn & adolescents (immediate reversal); term newborn infants & infants.

Adverse Reactions

Airway complications (bucking against endotracheal tube, coughing, bucking, arousal reaction or contra breath), anesth complications (movement of the limb or body, coughing, grimacing or suckling on the endotracheal tube), procedural complications (coughing, tachycardia, bradycardia & increased heart rate); recurrence of neuromuscular blockade; hypersensitivity reactions; marked bradycardia.

Drug Interactions

Delayed recovery time w/ toremifene (received on the same day of the operation). May cause displacement of rocuronium w/ fusidic acid (IV). Reduced effects & effectiveness of progestogen & OC steroids (either combined or progestogen only). Interference w/ serum progesterone assay.
