Diphenhydramine Hydrochloride.
Description
Benadryl (diphenhydramine hydrochloride) is 2-(diphenylmethoxy)-N-,N-dimethylamine hydrochloride, and occurs as a white, crystalline powder, and is freely soluble in water and alcohol.
Each 25 mg capsule contains 25 mg Diphenhydramine Hydrochloride.
Each 5 mL contains Diphenhydramine Hydrochloride 12.5 mg, alcohol 2%.
Action
Antihistamine.
Indications/Uses
For the temporary relief of: Hay fever and other upper respiratory allergy symptoms of runny nose, sneezing, itching of the nose or throat, and itchy, watery eyes; Common cold symptoms of runny nose and sneezing; Cough.
Treatment of allergic dermatoses such as urticaria, food allergy, pruritus.
Prevention and treatment of nausea, vomiting, or dizziness associated with motion sickness.
Helps to reduce difficulty falling asleep (adults and children 12 years and older).
Dosage/Direction for Use
Adults and children 12 years and over: Antitussive: The total daily recommended dose of diphenhydramine is up to 168 mg taken in divided doses of up to 28 mg every 4 hours.
Antihistaminic/Antiemetic: The total daily recommended dose of diphenhydramine is 300 mg taken in divided doses no greater than 25 to 50 mg every 4 to 6 hours.
Nighttime Sleep Aid: Oral dosage is 50 mg at bedtime.
Children aged 6 to less than 12 years: Antitussive: The total daily recommended dose of diphenhydramine is up to 84 mg taken in divided doses of up to 14 mg every 4 hours.
Antihistaminic/Antiemetic: The total daily recommended dose of diphenhydramine is 150 mg taken in divided doses no greater than 12.5 to 25 mg every 4 to 6 hours.
Children aged 4 to less than 6 years: Antitussive: The total daily recommended dose of diphenhydramine is 42 mg taken in divided doses of up to 7 mg every 4 hours.
Diphenhydramine, when indicated as an antitussive, is not recommended for use in children under 4 years of age.
Children aged 2 to less than 6 years: Antihistaminic: The total daily recommended dose of diphenhydramine is 37.5 mg taken in divided doses no greater than 6.25 mg every 4 to 6 hours.
Children under 2 years: Antihistaminic: Do not use in children less than 2 years of age unless directed by a physician.
What should the patient do if they miss a dose: Continue medication based on dosage and/or consult the doctor.
Overdosage
Mild to Moderate Symptoms: Somnolence, anticholinergic syndrome (mydriasis, flushing, fever, dry mouth, urinary retention, decreased bowel sounds), tachycardia, mild hypertension, nausea and vomiting are common after overdose. Agitation, confusion and hallucinations may develop with moderate poisoning.
Severe Symptoms: Effects may include delirium, psychosis, seizures, coma, hypotension, QRS widening, and ventricular dysrhythmias, including torsades de pointes, but are generally only reported in adults after large ingestions. Rhabdomyolysis and renal failure may rarely develop in patients with prolonged agitation, coma or seizures. Death may occur as a result of respiratory failure or circulatory collapse.
What to do when the patient has taken more than the recommended dosage: In the event of overdose, get medical help or contact a poison control center right away.
Contraindications
Hypersensitivity to diphenhydramine or to any of the ingredients.
Special Precautions
Patients with the following conditions should be advised to consult a physician before using diphenhydramine: A respiratory condition such as emphysema (adult products), chronic bronchitis, or acute or chronic bronchial asthma; glaucoma, prostate hyperplasia with urinary retention.
Do not use with any other product containing diphenhydramine, even one used on skin.
May cause drowsiness.
Diphenhydramine may enhance the sedative effects of central nervous system, depressants including alcohol, sedatives, and tranquilizers. While taking this product, avoid alcoholic beverages and consult a healthcare professional prior to taking with central nervous system depressants.
Specific for antitussive indication: Patients should not use this product for persistent or chronic cough, such as those occurring with asthma, or where cough is accompanied by excessive secretions, unless directed by a physician.
If symptoms persist or get worse, or if new symptoms occur, patients should stop use and consult a physician.
Effects on Ability to Drive or Use Machines: Use caution when driving a motor vehicle or operating machinery.
When using to reduce difficulty in falling asleep, do not drive a motor vehicle or operate machinery.
Use In Pregnancy & Lactation
There are no adequate and well-controlled clinical studies of diphenhydramine in pregnant or breast-feeding women. Diphenhydramine crosses the placenta and is excreted into breast milk, but levels have not been reported.
The product should not be used during pregnancy or lactation unless the potential benefit of treatment to the mother outweighs the possible risks to the developing fetus or breastfeeding infant.
Ask a physician before use if the patient is pregnant or breastfeeding.
Adverse Reactions
Post Marketing Data: Adverse drug reactions (ADRs) identified during post-marketing experience with diphenhydramine are confusional state, irritability, nervousness, hallucination, sedation, agitation, insomnia, coordination abnormal, convulsion, headache, paraesthesia, tremor, vision blurred, tinnitus, palpitations, tachycardia, dry throat, hypotension, chest discomfort, nasal dryness, constipation, diarrhoea, dyspepsia, nausea, vomiting, rash, pruritus, urticaria, urinary retention.
Drug Interactions
CNS depressants (alcohol, sedatives, tranquilizers): Diphenhydramine may enhance the sedative effects of central nervous system depressants, including alcohol, sedatives and tranquilizers.
Storage
Store at temperatures not exceeding 30°C.
