Tetanus antitoxin
Indications/Uses
Provides temporary passive immunity against tetanus. Patients w/ tetanus symptoms, given immediately together w/ surgical & other clinical remedies. Prophylactic inj for deep open wounds & those in danger of being infected.
Dosage/Direction for Use
SC/IM Around deltoid muscle of upper arm for SC inj while center area of deltoid muscle or lateral upper part of gluteus maximus for IM inj. Prophylactic use 1,500-5,000 IU to be given at once. Adult & childn Repeat inj after 6 days when contamination still persists. Patient previously immunized w/ tetanus toxoid Give a booster dose of tetanus toxoid only. Patient w/ previous inj of tetanus toxoid Give ≥1 inj of tetanus toxoid (but not tetanus antitoxin). Patient who has not had previous tetanus toxoid inj or w/o clear history of immunization Give both antitoxins for prophylaxis & permanent immunity. IV Use until no untoward reaction occurs after IM or SC inj. Slow IV inj not >1 mL/min & must not exceed 4 mL/min afterwards. Total vol for single dose: Adult Not >40 mL. Childn Not >0.8 mL/kg.
Contraindications
Special Precautions
Obtain information whenever possible as to whether previous inj of antiserum have been received & whether patient is subject to hypersensitivity disorders before injecting antiserum. Perform sensitivity testing before administration. Keep patient under observation after administration of doses of antisera. Adrenaline inj & resuscitation facilities should always be available/at hand. Give adrenaline at once in case of anaphylaxis. Handle all patients who developed hypersensitive reactions following inj properly.
Adverse Reactions
Anaphylaxis shock (eg, gloominess or dysphoria, pale or flush face, chest depression or asthma, cold sweat, nausea or abdominal pain, weak & rapid pulses, HTN in severe cases). Serum sickness (eg, urticaria, high fever, lymphadenopathy, local swelling & occasionally albuminuria, vomiting, joint pain including erythema, itchiness & edema at vaccination site).
