Amiodarone HCl
Indications/Uses
Prevention of tachyarrhythmias associated w/ Wolf-Parkinson-White syndrome & other types of tachyarrhythmias of paroxysmal nature, including supraventricular, nodal & ventricular tachycardia, atrial flutter & atrial fibrillation & ventricular fibrillation.
Dosage/Direction for Use
Individualized dosage. Adult Initial stabilization: Initially 200 mg tid for 1 wk, then reduce to 200 mg bid. Maintenance: 200 mg daily. Childn PO Loading dose: 10-20 mg/kg/day (500 mg/m2/day) for 7-10 days. Maintenance: Individualized dosage. 5-10 mg/kg/day (250 mg/m2/day). IV Loading dose: 5 mg/kg over 20 min to 2 hr. Maintenance: 10-15 mg/kg/day few hrs to several days. If needed, oral therapy may be initiated concomitantly at the usual loading dose.
Administration
Should be taken with food.
Contraindications
Sinus bradycardia & SA heart block, evidence or history of thyroid dysfunction. Avoid in patients w/ severe resp failure. High dosage may lead to severe bradycardia & to conduction disturbances w/ appearance of idioventricular rhythm in elderly or during digitalis therapy. Pregnancy & lactation.
Special Precautions
Patients w/ heart failure. Not to be used in patients w/ galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Intended only for patients w/ life-threatening arrhythmias. Monitor liver function before treatment & periodically thereafter.
Adverse Reactions
Impaired visual acuity; bradycardia or sinus arrest; photosensitization, bluish discoloration of skin; hyper- & hypothyroidism; severe pulmonary toxicity including fibrosis & interstitial pneumonitis, diffuse pulmonary alveolitis sometimes presenting as unexplained or disproportionate dyspnea; acute liver disorders w/ elevated serum transaminase &/or jaundice may occur; peripheral neuropathy &/or myopathy, nightmares, vertigo, headaches & sleeplessness, tremor & ataxia, benign raised ICP; ECG changes, QT interval lengthening corresponding to prolonged repolarization, U waves & deformed T waves; nausea, vomiting, metallic taste, fatigue & epididymo-orchitis.
Drug Interactions
Potential interaction w/ substrates, inhibitors or inducers of CYP3A4 & P-gp.
