Indications and Dosage
Oral
Cystitis
Adult: For relief of symptoms in women: As 4 g sachet for oral solution: 1 sachet (4 g) tid.

Parenteral
Anticoagulant
Adult: For use with automated apheresis devices: As 4% trisodium citrate dihydrate solution: Recommended dose is determined by the apheresis device. Refer to local or specific product guidelines.

Administration
Sodium citrate Should be taken with food. Swallow whole.
Reconstitution
Sachet for oral solution: Dissolve sachet contents in 125 mL of water.
Contraindications
Oral: Diabetes, heart disease, hypertension, history of renal disease or patients who are on low salt diet.
Special Precautions
Patient with shock or hypoperfusion. Hepatic impairment. Pregnancy and lactation.
Adverse Reactions
Cardiac disorders: Arrhythmia, chest pressure.
Gastrointestinal disorders: Stomach cramps.
Renal and urinary disorders: Mild diuresis (oral).
Vascular disorders: Hypotension.
Monitoring Parameters
When used as anticoagulant: Monitor serum electrolytes, acid-base balance and anion gap.
Overdosage
Symptoms: Gastrointestinal discomfort, diarrhoea, sodium overloading and hyperosmolality. Management: Symptomatic treatment. Correct fluid and electrolyte balance.
Drug Interactions
May reduce the plasma levels of lithium.
Action
Description:
Mechanism of Action: Sodium citrate is metabolised to bicarbonate which increases free bicarbonate ions excretion, resulting in increased urinary pH. Elevation in urinary pH enhances the solubility of cystine in the urine and the ionisation of uric acid to more soluble urate ion. Additionally, it acts as an anticoagulant by chelating free ionised calcium in the blood, thus making it unavailable for use in the coagulation system.
