Sacubitril + Valsartan - Oral

Why do I need this medicine?
Sacubitril and Valsartan are combined in this medicine.

Sacubitril + Valsartan is used to treat heart failure (weakness and inability of the heart to pump blood).
How do I take this medicine?
Take Sacubitril + Valsartan exactly as directed by your doctor or according to the instructions on the label. Do not take more or less than instructed by your doctor.

You may take this medicine with or without food. Try to take it at the same time each day.

Sacubitril + Valsartan is available as a tablet or as granules or pellets in a capsule.

If you are taking the tablet, swallow it whole with a glass of water. Do not chew or crush the tablet.

If you are taking the granules or pellets in a capsule, open the capsule and sprinkle the contents onto 1 to 2 teaspoons of soft foods then consume the mixture immediately. Do not swallow the capsule or empty capsule shell. Do not chew or crush the granules or pellets.

The dose of this medicine will be decided by your doctor. Your doctor will advise you on the treatment timeframe depending on your condition and response to the medication.

Sacubitril + Valsartan must be taken regularly for it to be effective. Continue taking this medicine even when you feel better. Do not stop taking it unless instructed by the doctor.
What should I do if I have forgotten to take this medicine?
Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and return to your normal dosing schedule.

DO NOT double a dose under any circumstances.

If you often forget to take your medicine, let your doctor and pharmacist know.
When should I not use this medicine?
Alert your doctor if you have the following conditions:
  • history of angioedema (swelling of the deep layers of the skin usually around the face, lips, and throat) while taking certain types of medicines for high blood pressure called angiotensin-converting enzyme (ACE) inhibitors or angiotensin II receptor blockers (ARB)
  • inherited angioedema or angioedema due to an unknown cause
  • biliary cirrhosis (a condition in which the bile ducts in the liver become slowly damaged)
  • cholestasis (impaired bile flow from the liver)
  • severe liver disease
as Sacubitril + Valsartan may not be suitable for you.

Do not take this medicine if you are pregnant. If you become pregnant while on Sacubitril + Valsartan therapy, alert your doctor immediately. This medicine may cause harm to your unborn child.

Do not take Sacubitril + Valsartan if you are currently taking or have taken a certain type of medicine for high blood pressure called ACE inhibitor such as enalapril, lisinopril and captopril in the last 36 hours.

Do not take this medicine with aliskiren (medicine to treat high blood pressure) if you have diabetes or kidney disease.
What should I take note of while taking this medicine?
Inform your doctor if you have the following conditions:
  • low sodium levels in the blood
  • low volume of circulating fluids in the body
  • low blood pressure
  • other heart problems e.g. narrowing of the valves of the heart
  • diabetes
  • hypoaldosteronism (a condition wherein the body has low levels of a hormone called aldosterone)
  • high potassium levels in the blood
  • moderate to severe kidney disease
  • moderate liver disease
Let your doctor know if you are breastfeeding.

Be sure to check with your doctor or pharmacist before giving Sacubitril + Valsartan to a child. Children may be more sensitive to the side effects.

If you are of a Black race, this medicine may be less effective. Inform your doctor if you are taking Sacubitril + Valsartan.

If you are going to have an operation, including minor surgery and dental work, inform your doctor or dentist that you are taking this medicine.

Why is it important to keep my appointments with the doctor?

Keep your appointments with your doctor. Your doctor needs to monitor your condition and check your response to the medication regularly.
  • Routine tests (e.g. kidney function, blood potassium levels) may be done while you are being treated with this medicine. Your doctor will advise you about how often you need to have these tests.
  • Your blood pressure may fall to unsafe levels without you noticing it. Monitor your blood pressure regularly during treatment.
  • Regular monitoring for signs and symptoms of angioedema may also be needed.
What side effects could I experience?
Sacubitril + Valsartan may cause any of the following side effects: nausea, diarrhoea, headache, dizziness with or without sensations of spinning, generalised weakness or lack of energy, and cough.

This medicine may cause your blood pressure to fall suddenly when you get up from a sitting or lying down position and you may feel giddy. To minimise this problem, stand up slowly.

Some side effects may need immediate medical help. Alert your doctor quickly if you experience any of the following:
  • swelling of the face, tongue, throat and/or lips
  • pain or difficulty in passing urine, change in how much urine is passed, blood in the urine
  • muscle weakness, cramping or numbness; fast or irregular heartbeat
  • hallucinations (seeing, hearing or feeling things that are not existent)
  • changes in sleeping pattern
Inform your doctor if any of these side effects do not go away or are severe, or if you experience other side effects.
Can I take this with other medicines?
Do not take Sacubitril + Valsartan if you are currently taking or have taken a certain type of medicine for high blood pressure called ACE inhibitor such as enalapril, lisinopril and captopril in the last 36 hours.

Do not take this medicine with aliskiren (medicine for high blood pressure) if you have diabetes or kidney disease.

Inform your doctor and pharmacist if you are taking any of these medicines:
  • medicines used to treat impotence e.g. sildenafil, tadalafil
  • cholesterol-lowering medicines e.g. atorvastatin
  • water pills or medicines for water retention e.g. spironolactone, triamterene, amiloride
  • NSAIDs (medicines for pain and inflammation) e.g. ibuprofen, celecoxib
  • ritonavir (medicine for HIV infection)
  • rifampicin (medicine to treat TB [lung infection known as tuberculosis])
  • ciclosporin (medicine used in organ transplants or certain immune disorders)
  • potassium supplements
  • lithium (medicine for mood disorder)
  • heparin (blood-thinning medicine)
This list does not include all medicines that may interact with Sacubitril + Valsartan.

Always notify your doctor and pharmacist if you are taking any other medicines, including herbal tonics such as traditional Chinese medicines, supplements, and medicines that you buy without a prescription.
What special dietary instructions should I follow?
Avoid alcohol.

Do not take salt substitutes that contain potassium.

Avoid taking too much food that is high in potassium, such as orange juice, bananas, or prunes.
How should I store this medicine?
Store in a cool, dry place away from the reach of children.

Protect from moisture.

Medicines must not be used past the expiry date.
