Phentermine (ion-exchange resin complex)
Indications/Uses
Short-term adjunct for obesity management in medically monitored comprehensive regimen of wt reduction based on exercise, diet & behaviour modification in obese (BMI ≥30 kg/m2) or overwt (BMI 25-29.9 kg/m2) patients w/ comorbidity.
Dosage/Direction for Use
Adult & childn ≥12 yr 1 cap daily. Recommended starting dose: 30 mg daily. Recommended continuous or intermittent maintenance dose: 15-30 mg daily depending on responsiveness.
Administration
Should be taken with food: Take at breakfast. Avoid taking near bedtime. Swallow whole.
Contraindications
Known hypersensitivity to sympathomimetic drugs. Pulmonary artery HTN; existing heart valve abnormalities or heart murmurs; moderate to severe arterial HTN; cerebrovascular disease; severe cardiac disease including arrhythmias, advanced arteriosclerosis; hyperthyroidism; agitated states or history of psychiatric illness including anorexia nervosa & depression; glaucoma; history of drug/alcohol abuse or dependence. Concomitant use w/ MAOIs or w/in 14 days of administration.
Special Precautions
Valvular heart disease; primary pulmonary HTN. Mild HTN, diabetes, established CAD, epilepsy, patients receiving psychotropic drugs including sedatives & sympathomimetics, antihypertensives. Require medical review at least every 3 mth. Wt loss should be gradual & controlled. Not to be combined w/ other appetite suppressants. Not recommended w/ drug products for wt loss; SSRIs, ergot-like drugs & clomipramine. Concomitant use w/ fenfluramine or dexfenfluramine. May affect ability to drive & use machines. Not to be used during pregnancy. Avoid use during lactation. Not recommended in childn <12 yr & elderly.
Adverse Reactions
Palpitations, tachycardia, elevated BP, precordial pain. Overstimulation, restlessness, nervousness, insomnia, tremor, dizziness, headache; nausea, vomiting, dry mouth, abdominal cramps, unpleasant taste, diarrhoea, constipation; micturition disturbances, rash, impotence, changes in libido, facial oedema.
Drug Interactions
Antagonised adrenergic neurone blockers eg, clonidine, methyldopa, guanethidine. Potentiated effects by MAOIs. Increased CNS stimulation w/ thyroid hormones. Increased CNS side effects w/ alcohol. Caution use w/ sympathomimetics.
