Ciprofloxacin

Resp tract, eyes, kidneys, efferent urinary tract, abdominal cavity (eg, biliary tract, peritonitis), skin & soft tissues, bones & joints infections. Genital infections including adnexitis, gonorrhoea, prostatis. Prophylaxis in immunosuppressed patients or w/ neutropenia. Acute bacterial rhinosinusitis, nosocomial or hospital-acquired pneumonia, acute otitis media, septicemia, uncomplicated cystitis or acute cystitis.

Resp tract infection 250-500 mg bd. Acute, uncomplicated UTI 125 mg bd or 250 mg once-bd. Pre-menopausal cystitis & acute, uncomplicated gonorrhoea 250 mg as single dose. Complicated UTI 250-500 mg bd. Extragenital gonorrhoea 125 mg bd. Diarrhea 500 mg once-bd. Other infections 500 mg bd. Particularly severe, life-threatening Strep pneumonia, recurrent infections in cystic fibrosis, bone & joint infections, septicaemia, peritonitis, especially when Pseudomonas, Staph or Strep is present 750 mg bd. Duration: Kidney, urinary tract & abdominal cavity infections Up to 7 days. Osteomyelitis Max: 2 mth. Other infections 7-14 days. Strep or Chlamydia infections At least 10 days. CrCl 31-60 mL/min/1.73 m² Max: 1,000 mg daily, CrCl ≤30 mL/min/1.73 m² Max: 500 mg daily.

May be taken with or without food: May be taken w/ meals. Swallow whole w/ small amount of fluid.

Hypersensitivity to ciprofloxacin or other quinolones. Pregnancy & lactation. Childn & adolescents 5-17 yr (except association w/ cystic fibrosis).

Discontinue use at 1st sign of tendinitis & if psychiatric reactions develop. Avoid use in patients w/ history of serious reactions to fluoroquinolone-containing products & myasthenia gravis. Epilepsy, CNS disorders; history of aneurysm disease, pre-existing aortic aneurysm &/or aortic dissection, presence of other risk factors or conditions predisposing for aortic aneurysm & dissection (eg, Marfan syndrome, vascular Ehlers-Danlos syndrome, Takayasu arteritis, giant cell arteritis, Behcet's disease, HTN, known atherosclerosis); peripheral neuropathy; psychotic patients or history of psychiatric disease. G6PD deficiency, crystalluria; patients w/ solid organ transplants. Avoid prolonged exposure to sunlight or UV radiation. Avoid concomitant use of corticosteroids. Concurrent use w/ antacids, Al-, Ca-, Mg- or Fe-containing prep, anticoagulants (eg, warfarin), pain & inflammation relievers (except aspirin), glibenclamide, probenecid or metoclopramide, cyclosporin, phenytoin. May affect ability to drive or operate machinery. Hepatic & renal impairment. Not recommended during pregnancy & lactation. Elderly >60 yr.

Diarrhoea, nausea, skin rashes. Exacerbation of myasthenia gravis.

Increased serum theophylline conc. Provoked convulsion w/ NSAIDs (except ASA). Transient raised serum creatinine conc w/ cyclosporin. Intensified action of warfarin, glibenclamide. Increased serum conc w/ probenecid. Increased plasma levels & associated toxic reactions of MTX. Reduced absorption w/ Fe, sucralfate or Mg-, Al- or Ca-containing antacids & highly buffered drugs (eg, didanosine tab). Accelerated absorption by metoclopramide.

Quinolones
