Fluconazole
Indications/Uses
Acute or recurrent genital & vag candidiasis. Candidal balanitis, dermatomycosis including tinea pedis, tinea corporis, tinea cruris, tinea versicolor, dermal candida infections.
Dosage/Direction for Use
Adult Vag candidiasis/candidal balanitis 150 mg single dose. Dermal infections including tinea pedis, corporis, cruris & candida infections 150 mg once wkly for 2-4 wk. Tinea pedis Up to 6 wk. Tinea versicolor 300 mg once wkly for 2 wk, 3rd wkly dose: 300 mg may be needed or 300-400 mg single dose.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to fluconazole or other-related azole compds. Co-administration w/ terfenadine or cisapride.
Special Precautions
Hypersensitivity. Discontinue use if clinical signs or symptoms of liver disease develops. Exfoliative cutaneous reactions eg, SJS, TEN; AIDS, cancer, haematological abnormalities; congenital or documented acquired QT prolongation, cardiomyopathy, sinus bradycardia, existing symptomatic arrhythmias, electrolyte disturbances eg, hypokalaemia, hypomagnesaemia, hypocalaemia. Concomitant use w/ medications not metabolized by CY34A but prolonging QT interval. Hepatic & renal dysfunction. Women of childbearing potential should use effective contraception during & approx 1 wk after last dose. Avoid use during pregnancy. Not recommended during lactation. Not recommended in childn <16 yr.
Adverse Reactions
Headache; rash; abdominal pain, diarrhoea, flatulence, nausea.
Drug Interactions
Decreased AUC & shortened t½ w/ rifampicin. Increased plasma conc w/ hydrochlorothiazide. Increased prothrombin time w/ warfarin. Increased midazolam, cyclosporin conc. Prolonged serum t½ of sulphonylureas. Increased phenytoin, terfenadine, rifabutin, tacrolimus serum plasma levels. Increased AUC of ethinyloestradiol, levonorgestrel, zidovudine. Decreased theophylline mean plasma clearance. Cardiac events eg, Torsade de pointes w/ cisapride. Elevated serum levels of CYP450-metabolised drugs.
