As monotherapy or adjunct therapy in the treatment of (elementary or complex) partial seizures and absence seizures (petit mal seizures).
Recommended initial dose: 15 mg/kg/day, which can be increased at weekly interval by 5-10 mg/kg/day until seizures are controlled or any side effects occur that prevent further treatment. Maximum dose: 60 mg/kg/day. If total daily dose is >250 mg, it should be given in divided doses. To be taken with food.
Hypersensitivity. Patients with liver disease or those showing significant liver dysfunction; patients with known mitochondrial disorders due to mitochondrial DNA polymerase gamma (POLG) gene (e.g. Alpers Huttenlocher syndrome) mutation and children less than 2 years old with suspected POLG-related disorders; patients with systemic primary carnitine deficiency with uncorrected hypocarnitinemia.
May cause fatal hepatotoxicity or liver dysfunction; Patients with hereditary neurometabolic syndrome caused by mutations in the gene for mitochondrial DNA polymerase gamma (POLG). Increased risk of suicidal behavior or thought. Women with childbearing potential or pregnancy. Children <2 years old. Elderly (>65 years old). May impair the ability to drive the vehicle or operate the machinery.

Concomitant use with ritonavir, phenytoin, carbamazepine, phenobarbital, primidone, aspirin, carbapenem antibiotics, colestyramine, oestrogen-containing hormonal contraceptive, felbamate, metamizole, methotrexate, lopinavir, rifampicin, amitriptyline/nortriptyline, carbamazepine, carbamazepine-10, 11-Epoxide, clonazepam, diazepam, ethosuximide, lamotrigine, cefditoren pivoxil, adefovir dipivoxil, pivmecillinam, pivampicillin, primidone, propofol, nimodipine, tolbutamide, cannabidiol, topiramate, acetazolamide, warfarin, zidovudine, quetiapine, lorazepam, olanzapine, rufinamide, steroid-containing oral contraceptives.
