Serious infections of lower resp tract, urinary tract including uncomplicated gonorrhea, skin & soft tissue, intra-abdominal including peritonitis, bone & joints, CNS eg, meningitis; gynecological infections including pelvic inflammatory disease, endometritis & pelvic cellulitis, septicemia, typhoid fever; peri-op prophylaxis.
Dosage/Direction for Use
Adult & childn ≥12 yr 1 g 12 hrly. Moderate to severe 1-2 g IM or IV 6-8 hrly. Life-threatening infection 2 g IV 4 hrly. Severe infection Max: 12 g/day. Uncomplicated gonorrhea 1 g IM. Patient w/ CrCl <20 mL/min Reduce dose by ½ the normal. Infant & childn 1 mth-12 yr, >50 kg Adult dose; <50 kg 50-180 mg/kg in 4-6 equal doses. Neonate 1-4 wk 50 mg/kg IV 8 hrly, 0-1 wk 50 mg/kg IV 12 hrly. Peri-op prophylaxis 1-2 g 30-90 min pre-op. Duration of treatment: 48-72 hr after the patient defervescence or bacterial eradication has been obtained. Group A β-hemolytic streptococci Min: 10 days.
Contraindications
Hypersensitivity to cephalosporins.
Special Precautions
Hypersensitivity to penicillins; history of GI disturbances especially colitis; concurrent treatment w/ aminoglycoside; long-term treatment; neutropenia (discontinue). False +ve results in Coombs' & glycosuria tests. Pregnancy (especially 1st trimester) & lactation.
Adverse Reactions
Local & hypersensitivity reactions; GI disturbances; hematological reactions; moniliasis, vaginitis; headache. Temporary elevation of hepatic enzymes, BUN or serum creatinine; encephalopathy (high doses); Herxheimer reaction (spirochaete infections).
