Indications/Uses
Inj: Prophylaxis & treatment of venous thrombosis & pulmonary embolism. Treatment of arterial embolism. Prevention of coagulation in heart & arterial surgery, cerebral thrombosis. Anticoagulant in blood transfusions, extracorporeal circulation dialysis & needs lab. Topical gel: Superficial phlebitis, excessive vena blockage, injuries during exercise & accident.
Dosage/Direction for Use
Inj Hemodialysis Normal dose: 7,500-12,500 IU; IV 5,000-10,000 IU 4 hrly by bolus & continuous infusion in NaCl or dextrose. SC (therapeutic dose) 10,000 IU 8 hrly after bolus analysis IV 5,000 IU. Low-dose heparin regimen Patient >40 yr undergoing abdominal or thoracic surgery 5,000 IU SC 2 hr pre-op & continued for 7 days 8-12 hrly w/o lab control. Topical Gel Apply thinly in the affected area 2-3 times daily. May be applied in the gauze then cover the affected area.
Contraindications
Hypersensitivity. Inj: Haemorrhagic diatheses, GI tract ulcers, severe HTN (decompensated), CNS surgical procedures, suspected brain haemorrhage, endocarditis lenta, thyroid operation, suspected malignant tumor w/ risk of bleeding. Not to be given IM for possibility of hematoma. Severe liver, kidney & pancreatic disease as well as imminent abortus in anticoagulant therapy.
Special Precautions
Inj: Hypersensitivity. Not for IM administration. Patients w/ disruptive ulcerative lesions, menstruation or hemostatic liver disease. Patients who will undergo surgery & post-op. Discontinue if coagulation test is too long or bleeding occurs. Closely monitor for thrombocytopenia. Patients w/ cardiac arrhythmia given higher doses of benzyl alcohol. Pregnancy. Elderly. Topical Gel: Not to be applied in mucous membranes or open wounds. Not to be rubbed or massaged in areas w/ thrombosis & thrombophlebitis condition.
Adverse Reactions
Inj: Local or generalized allergic reactions. Transient & reversible hair loss, thrombocytopenia, osteoporosis (long-term therapy). Headache, nausea, itching & rise in body temp in anticoagulant therapy. Occurrence of hematoma in prophylaxis of thromboembolism; bleeding in the wound can increase. Haemorrhage manifestation, increased transaminase enzyme & hypereosinophilia.
Drug Interactions
Inj: Synergistic effect w/ ASA, dipirydamole, phenylbutazone, indometacin, sulfinpyrazone, probenecid, dextran, ethacrynic acid, dicumarol, cytostatic drugs, aspirin, NSAID, octylsalicylic acid, hydrochloroquine. Counteractive effect w/ antihistamines, digitalis prep, tetracycline, nicotine abuse, ascorbic acid.
