Glyceryl trinitrate
Indications/Uses
Acute angina attacks. Angina prophylaxis. Acute heart attacks. Acute heart failure, w/ acute left heart failure. Catheter-induced coronary spasms during coronary angiography.
Dosage/Direction for Use
SL Start of angina attack or immediately before stress known to trigger attack 0.4-1.2 mg (1-3 puffs), depending on severity. Acute heart failure & acute heart attack 0.4-1.2 mg (1-3 puffs), depending on severity, w/ monitoring of blood circulation (systolic BP >100 mmHg). Repeat same dose if no response after 10 min. Preventive measure before coronary angiography 0.4-0.8 mg (1-2 puffs).
Contraindications
Hypersensitivity to glyceryl trinitrate, other nitrate compd, or peppermint oil. Acute circulatory failure. Marked hypotension (systolic BP <90 mmHg). Cardiogenic shock, unless appropriate measures are in place to ensure a sufficiently high left-ventricular end diastolic pressure. Hypertrophic obstructive cardiomyopathy. Disorders associated w/ increased ICP (eg, head injuries, brain bleed). Severe anaemia. Concomitant use w/ phosphodiesterase-5 inhibitors.
Special Precautions
Do not inhale. Caution before using in patients w/ constrictive pericarditis or pericardial tamponade; low filling pressure eg, in acute heart attack, left-sided heart failure; aortic &/or severe mitral valve stenosis; risk of orthostatic hypotension; disorder affecting cerebral blood flow; early stages of glaucoma; left ventricular hypertrophy combined w/ aortic valve stenosis. Contains small amounts of ethanol (<10 mg/dose). May impair ability to drive or operate machinery. Pregnancy & lactation. No data in childn.
Adverse Reactions
Nitrate-induced headache; decreased BP &/or orthostatic hypotension.
Drug Interactions
Increased antihypertensive effect w/ other vasodilators; antihypertensives (eg, β-blockers, Ca antagonists, diuretics, ACE inhibitors); TCAs or neuroleptics; alcohol; sapropterin; phosphodiesterase-5 inhibitors (eg, sildenafil, vardenafil). Enhanced vasodilatory effect w/ N-acetylcysteine. Patients previously treated w/ organic nitrates (eg, isosorbide dinitrate, isosorbide-5-mononitrate) may require higher dose of glyceryl trinitrate to achieve desired effect. Raised levels of dihydroergotamine. Weakened effect of heparin.
