Emetogenic chemotherapy & radiotherapy Adult 8 mg PO 1-2 hr before chemotherapy/radiation followed by 8 mg PO every 12 hr later for a max of 5 days.
Highly emetogenic chemotherapy (eg high-dose cisplatin) Adult 8 mg as single dose by slow IV/IM immediately before chemotherapy followed by 2 further IV doses of 8 mg 2-4 hr apart or by a constant infusion of 1 mg/hr for up to 24 hr. Doses >8-16 mg may only be given by IV infusion diluted in 50-100 mL saline or other compatible infusion fluid & infused over not <15 min immediately before chemotherapy. A single dose >16 mg should not be given. Efficacy may be enhanced by addition of dexamethasone Na phosphate 20 mg as single IV dose prior to chemotherapy. To protect against delayed or prolonged emesis after the 1st 24 hr, continue w/ 8 mg PO bd for 5 days.
Chemotherapy-induced nausea & vomiting (CINV) Childn & adolescent ≥2 yr (BSA 0.6-1.2 m2) 5 mg/m
2 as single IV dose immediately before chemotherapy, followed by 4 mg PO 12 hr later. Continue w/ 4 mg PO bd for up to 5 days after a course of treatment.
Radiotherapy-induced nausea & vomiting (RINV) & CINV Elderly No alteration of oral dose or frequency is required.
PONV Adult Prevention: 16 mg PO 1 hr prior to anaesth.
