Indications/Uses
Invasive aspergillosis; candidaemia in non-neutropenic patients; fluconazole-resistant serious invasive Candida infections (including C. krusei); serious fungal infections caused by Scedosporium spp & Fusarium spp. Prophylaxis in patients ≥12 yr at high risk of developing invasive fungal infections.
Dosage/Direction for Use
Adult Oral Loading dose (1st 24 hr): Patient ≥40 kg 400 mg 12 hrly, <40 kg 200 mg 12 hrly. Maintenance dose (after 1st 24 hr): Prophylaxis of invasive fungal infections, serious invasive Candida/invasive aspergillosis/Scedosporium & Fusarium infections, candidaemia in non-neutropenic patient Patient ≥40 kg 200 mg 12 hrly, <40 kg 100 mg 12 hrly. Co-administration w/ phenytoin If maintenance dose is increased from 200 mg to 400 mg 12 hrly. Patient <40 kg If maintenance dose is increased from 100 mg to 200 mg 12 hrly. Co-administration w/ adjusted doses of efavirenz Increase maintenance dose to 400 mg 12 hrly. IV Loading dose (1st 24 hr): 6 mg/kg 12 hrly. Maintenance dose (after 1st 24 hr): Prophylaxis of invasive fungal infections & candidaemia in non-neutropenic patient 3-4 mg/kg 12 hrly. Serious invasive Candida/invasive aspergillosis/Scedosporium & Fusarium infections 4 mg/kg 12 hrly. Co-administration w/ phenytoin If the maintenance dose is increased to 5 mg/kg 12 hrly. Adolescent (12-16 yr) Adult dose. Childn 2 to <12 yr Loading dose: No oral or IV loading dose is recommended. Maintenance dose: Oral 200 mg bd. IV 7 mg/kg bd.
Administration
Should be taken on an empty stomach: FC tab: Take at least 1 hr before or after meals. Oral susp: Take at least 1 hr before or 2 hr after meals.
Contraindications
Hypersensitivity. Co-administration w/ CYP3A4 substrates, terfenadine, astemizole, cisapride, pimozide, quinidine or ivabradine; sirolimus; rifabutin, rifampicin, carbamazepine & long-acting barbiturates (eg, phenobarb); St. John's wort; efavirenz (≥400 mg once daily); ergot alkaloids (ergotamine, dihydroergotamine); high-dose ritonavir (≥400 mg bd); naloxegol; tolvaptan; venetoclax; lurasidone; finerenone.
