Adult Oral Loading dose (1st 24 hr):
Patient ≥40 kg 400 mg 12 hrly,
<40 kg 200 mg 12 hrly. Maintenance dose (after 1st 24 hr):
Prophylaxis of invasive fungal infections, serious invasive Candida/invasive aspergillosis/Scedosporium & Fusarium infections, candidaemia in non-neutropenic patient Patient ≥40 kg 200 mg 12 hrly,
<40 kg 100 mg 12 hrly.
Co-administration w/ phenytoin If maintenance dose is increased from 200 mg to 400 mg 12 hrly.
Patient <40 kg If maintenance dose is increased from 100 mg to 200 mg 12 hrly.
Co-administration w/ adjusted doses of efavirenz Increase maintenance dose to 400 mg 12 hrly.
IV Loading dose (1st 24 hr): 6 mg/kg 12 hrly. Maintenance dose (after 1st 24 hr):
Prophylaxis of invasive fungal infections & candidaemia in non-neutropenic patient 3-4 mg/kg 12 hrly.
Serious invasive Candida/invasive aspergillosis/Scedosporium & Fusarium infections 4 mg/kg 12 hrly.
Co-administration w/ phenytoin If the maintenance dose is increased to 5 mg/kg 12 hrly.
Adolescent (12-14 yr weighing <50 kg) &
childn 2 to <12 yr Loading dose (1st 24 hr):
Oral Not recommended.
IV 9 mg/kg every 12 hr. Maintenance dose:
Oral FC tab: 9 mg/kg bd. Max: 350 mg bd). Oral susp: 0.225 mL/kg (9 mg/kg) bd. Max: 8.75 mL (350 mg) bd.
IV 8 mg/kg bd.
Adolescent 12-14 yr, ≥50 kg; 15-16 yr regardless of body wt Adult dose.
Prophylaxis in adult & childn Initiated on day of transplant for up to 100 days.
Continuing immunosuppression or graft versus host disease May continue up to 180 days after transplant.
