Indications/Uses
Adjunct to diet for reduction of elevated total-cholesterol (total-C), LDL-C, Apo B & triglycerides (TG) in adults, adolescents & childn ≥10 yr w/ primary hypercholesterolemia, heterozygous familial hypercholesterolemia or combined (mixed) hyperlipidemia (Fredrickson types IIa & IIb), elevated serum TG levels (Fredrickson type IV) & those w/ dysbetalipoproteinemia (Fredrickson type III) when response to diet & other non-pharmacological measures is inadequate. Raises HDL-C; lowers LDL/HDL & total-C/HDL ratios. Reduces total-C & LDL-C in adults w/ homozygous familial hypercholesterolemia as an adjunct to other lipid-lowering treatments (eg, LDL apheresis) or if such treatments are unavailable. Prevention of CV disease.
Dosage/Direction for Use
Individualized dosage. Primary hypercholesterolemia & combined (mixed) hyperlipidemia 10 mg once daily. Homozygous familial hypercholesterolemia May consider 80 mg daily. Prevention of CV disease 10 mg once daily for primary prevention. Optimal dose may range 10-80 mg once daily for secondary prevention. Paed patient ≥10 yr Recommended starting dose: 10 mg daily w/ titration up to 20 mg daily. Co-administration w/ cyclosporine, telaprevir, tipranavir/ritonavir Max: 10 mg. Concomitant use w/ elbasvir/grazoprevir Max daily dose: 20 mg.
Administration
May be taken with or without food: Avoid excessive consumption (>1.2 L/day) of grapefruit juice.
Contraindications
Hypersensitivity. Active liver disease or unexplained persistent elevations of serum transaminases >3 x ULN. Concomitant use w/ glecaprevir/pibrentasvir. Women of childbearing potential. Pregnancy & lactation.
