Epilepsy: Adjunctive therapy Adult & elderly Initially 25-50 mg nightly for 1 wk. Subsequently at wkly or bi-wkly intervals, increase dose to 25-50 mg/day in 2 divided doses. Usual daily dose: 200-400 mg in 2 divided doses.
Childn ≥2 yr 5-9 mg/kg/day in 2 divided doses. Titrate at 25 mg (or less based on range of 1-3 mg/kg/day) nightly for the 1st wk. Subsequently at 1- or 2-wk intervals, increase dose by increments of 1-3 mg/kg/day in 2 divided doses. Max: 30 mg/kg/day.
Monotherapy Adult & elderly Titrate at 25 mg nightly for 1 wk. Subsequently at 1- or 2-wk intervals, increase dose by increments of 25 or 50 mg/day in 2 divided doses. Initial target dose: 100 mg/day. Max: 500 mg daily.
Patient w/ refractory forms of epilepsy 1,000 mg/day.
Childn ≥2 yr Initially at 0.5-1 mg/kg nightly for the 1st wk, then increased at 1- or 2-wk interval by increments of 0.5-1 mg/kg/day in 2 divided doses. Initial target dose: 3-6 mg/kg/day.
Migraine Adult Titrate at 25 mg nightly for 1 wk. Increase by increments of 25 mg/day at 1-wk intervals. Total daily dose: 100 mg/day in 2 divided doses.
