Esketamine

Indications/Uses

In combination w/ an oral antidepressant (SSRI or SNRI) for treatment-resistant depression (major depressive disorder in adults who have not responded adequately to at least 2 different antidepressants of adequate dose & duration to treat the current depressive episode). Co-administration w/ oral antidepressants as acute short-term treatment in adults w/ moderate to severe episodes of major depressive disorder for the rapid reduction of depressive symptoms.

Dosage/Direction for Use

Treatment-resistant depression Adult Induction phase [wk 1-4 (2 treatment sessions/wk)] Starting dose: 56 mg (day 1). Subsequent doses: 56 or 84 mg. Maintenance phase 56 or 84 mg once wkly (wk 5-8), 56 or 84 mg every 2 wk or once wkly (from wk 9). Elderly ≥65 yr Induction phase [wk 1-4 (2 treatment sessions/wk)] Starting dose: 28 mg (day 1). Subsequent doses: 56 or 84 mg. Maintenance phase 56 or 84 mg once wkly (wk 5-8), 56 or 84 mg every 2 wk or once wkly (from wk 9). Subsequent doses should be increased in 28 mg increments up to 56 or 84 mg based on efficacy & tolerability. Acute short-term treatment of psychiatric emergency due to major depressive disorder Adult 84 mg twice wkly for 4 wk. Continue oral antidepressant after 4 wk per clinical judgement.

Contraindications

Hypersensitivity to esketamine or ketamine. Patients for whom an increase in BP or intracranial pressure poses a serious risk. Known aneurysmal vascular disease (including intracranial, thoracic, abdominal aorta or peripheral arterial vessels); history of intracerebral hemorrhage.
