Na valproate

Indications/Uses

Epileptic patients who would normally be maintained on oral Na valproate & for whom oral therapy is temporarily not possible. Generalized or partial epilepsy particularly w/ seizure patterns of absence, myoclonic, tonic-clonic, atonic & mixed. Partial epilepsy eg, simple, complex or secondary generalized seizures & specific syndromes (West, Lennox-Gastaut).

Dosage/Direction for Use

IV Epilepsy 400-800 mg depending on body wt up to 10 mg/kg inj over 3-5 min followed by continuous or repeated infusion up to max of 2,500 mg/day. Childn 20-30 mg/kg/day inj over 3-5 min followed by continuous or repeated infusion. Patient in whom plasma valproic acid level can be monitored May increased dose up to 40 mg/kg/day if adequate control is not achieved. Concomitant use w/ other anticonvulsant Gradually initiate therapy w/ target dose being reached after 2 wk. May raise Na valproate by 5-10 mg/kg/day when used in combination w/ liver enzyme-inducing anticonvulsants (eg, phenytoin, phenobarb & carbamazepine).

Contraindications

Hypersensitivity to Na valproate, divalproate or valpromide. Acute liver disease. Chronic hepatitis. Porphyria. Patients w/ known urea cycle disorders. Patients known to have mitochondrial disorders (eg, Alpers-Huttenlocher syndrome). Known systemic primary carnitine deficiency w/ uncorrected hypocarnitinaemia. Personal or family history of severe hepatic dysfunction particularly drug-induced. Women of childbearing potential. Pregnancy. Childn <2 yr who are suspected of having polymerase γ-related disorder.
