HTN Adult 4 mg once daily in the morning. May be increased to 8 mg once daily after 1 mth of treatment.
Patient w/ strongly activated renin-angiotensin-aldosterone system Initially 2 mg once daily. May be increased to 8 mg once daily after 1 mth of treatment.
Hypertensive patient in whom diuretic cannot be discontinued Initially 2 mg once daily.
Elderly Initially 2 mg once daily which may be progressively increased to 4 mg after 1 mth then 8 mg if necessary depending on renal function.
Symptomatic heart failure Initially 2 mg once daily in the morning. May be increased by increments of 2 mg at intervals of not <2 wk to 4 mg once daily if tolerated.
Stable CAD 4 mg once daily for 2 wk then increased to 8 mg once daily depending on renal function & provided that the 4 mg daily dose is well tolerated.
Elderly 2 mg once daily for 1 wk then 4 mg once daily the next wk before increasing dose up to 8 mg once daily depending on renal function.
Renal impairment: CrCl ≥60 mL/min 4 mg daily,
≥30-<60 mL/min 2 mg daily,
≥15-<30 mL/min 2 mg every other day,
<15 mL/min (haemodialysed patient) 2 mg on the day of dialysis.
