Indications/Uses
Induction of labor for medical reasons (eg, post-term gestation, premature rupture of membranes, pregnancy-induced HTN associated w/ significant protein in urine (pre-eclampsia). Labor enhancement in selected cases of uterine inertia. Adjunctive therapy for incomplete, inevitable or missed abortion in early stage of pregnancy. After delivery of child during cesarean section. Prevention & treatment of postpartum hemorrhage associated w/ uterine atony.
Dosage/Direction for Use
IV/IM Labor induction or enhancement 10 IU in 1,000 mL of electrolyte solution infused initially at 1-2 milliunits/min, gradually increase at intervals not <20 min & increments not >1-2 milliunits/min until contraction pattern similar to that of normal labor is established. Max rate: 20 milliunits/min. Cesarean section 5 IU infused immediately after delivery. Postpartum uterine hemorrhage prevention 5 IU by infusion or 5-10 IU IM after placenta delivery. Postpartum uterine hemorrhage treatment 5 IU by infusion or 5-10 IU IM followed in severe cases by 5-10 IU in 500 mL of electrolyte-containing diluent run at the rate necessary to control uterine atony. Incomplete, inevitable or missed abortion 5 IU by infusion or 5-10 IU IM followed by infusion at rate of 20-40 milliunits/min if necessary.
Contraindications
Hypersensitivity. Not to be administered w/in 6 hr after vag prostaglandins have been given. Not to be used for prolonged periods in patients w/ oxytocin-resistant uterine inertia, severe pre-eclamptic toxaemia or severe CV disorders. Hypertonic uterine contractions, fetal distress when delivery is not imminent. Any condition in which spontaneous labor is unadvisable &/or vag delivery is contraindicated (eg, significant cephalopelvic disproportion, fetal malpresentation, placenta previa, vasa previa, placental abruption, cord presentation or prolapse, overdistension or impaired uterus resistance to rupture as in multiple pregnancy, polyhydramnios, grand multiparity, uterine scar presence resulting from major surgery including classical cesarean section).
