Morphine sulphate
Indications/Uses
Relief of moderate to severe pain unresponsive to non-opioid analgesics. Pre-op medication & analgesic adjunct in general anaesth.
Dosage/Direction for Use
Adult 5-20 mg IM or SC, may be repeated every 4-6 hr; or 2.5-15 mg slow IV over 4-5 min if rapid onset of action is desired. Childn 0.1-0.2 mg/kg IM or SC to a max of 15 mg, may be repeated every 4-6 hr. May be titrated IV in a dose of 0.05-0.1 mg/kg incrementally over 5-15 min. Management of acute pain via IV infusion Adult w/ no history of opioid intake 0.5-2 mg/hr after adequate analgesia is established. Childn An infusion dose of 0.01-0.05 mg/kg/hr to a max IV dose of 4 mg/hr. Patient-controlled analgesia (PCA) Adult 0.5 mg to a max of 1.5 mg w/ a lockout interval of 6-10 min. Childn & adolescent 7-19 yr 0.01-0.025 mg/kg w/ a lockout interval of 6-10 min. An infusion dose of 0.015 mg/kg/hr may be used if a background infusion is employed.
Contraindications
Hypersensitivity to morphine or other opioids. Acute or severe bronchial asthma or other obstructive airway disease; severe resp disease, acute resp disease, resp insufficiency or depression especially in the presence of cyanosis &/or excessive bronchial secretion; other conditions where resp reserve is depleted (eg, severe emphysema, chronic bronchitis or kyposcoliosis); cor pulmonale; severe CNS depression; diabetic acidosis w/ risk of coma; incipient hepatic encephalopathy; post-biliary tract surgery or surgical anastomosis; biliary colic; GI obstruction, paralytic ileus; suspected surgical abdomen; acute diarrhoeal conditions associated w/ antibiotic-induced pseudomembranous colitis; diarrhoea caused by poisoning; pheochromocytoma; cardiac arrhythmias; heart failure secondary to pulmonary disease; acute alcoholism or delirium tremens; head injuries; brain tumour; raised intracranial or cerebrospinal pressure & in convulsive states (eg, status epilepticus, tetanus or strychnine poisoning). Severe liver disease. Concomitant or w/in 14 days of MAOI therapy. Continuous IV infusion in patients w/ hepatic or renal disease. Administration via PCA to childn <6 yr & adult w/ poor cognitive function. Premature infants or during labour for delivery of premature infants.
