Indications/Uses
Antineoplastic chemotherapy for gestational choriocarcinoma, chorioadenoma destruens & hydatidiform mole. Palliation of acute lymphocytic leukemia; acute lymphoblastic (stem cell) leukemia in childn. Treatment & prophylaxis of meningeal leukemia. Induction of remission in combination w/ other anticancer drugs or suitable agents. Monotherapy or in combination w/ other anticancer agents in management of breast cancer, epidermoid cancers of the head & neck, lung cancer particularly squamous cells & small cell types. Advanced stages (III & IV, Peters Staging System) of lymphosarcoma particularly in those cases in childn; advanced cases of mycosis fungoides. Psoriasis chemotherapy; symptomatic control of severe, recalcitrant, disabling psoriasis which is not adequately responsive to other forms of therapy but only when diagnosis has been established.
Dosage/Direction for Use
Choriocarcinoma & similar trophoblastic diseases 15-30 mg daily for 5-day course. Leukemia Administer 2 times wkly either PO, when remission is achieved & supportive care has produced general clinical improvement. Meningeal leukemia Give at intervals of 2-5 days. Burkitt's tumor stages I-II 10-25 mg daily for 4-8 days. Lymphosarcoma stage III 0.625-2.5 mg/kg daily. Mycosis fungoides 2.5-10 mg daily for wk or mth. Psoriasis chemotherapy Recommended starting dose sched: Wkly single oral: 10-25 mg wkly until adequate response is achieved. Max: 25 mg wkly. Divided oral dose sched: 2.5 mg every 12 hr for 3 doses each wk. Max: 30 mg wkly.
Administration
May be taken with or without food: Swallow whole w/ full glass of water. Do not crush/chew.
Contraindications
Hypersensitivity. Severe renal impairment. Lactation. Patient w/ psoriasis: Alcoholism, alcoholic liver disease or other chronic liver disease. Overt or lab evidence of immunodeficiency syndromes. Preexisting blood dyscrasias (eg, bone marrow hypoplasia, leukopenia, thrombocytopenia or anaemia). Pregnancy.
