Lidocaine HCl monohydrate
Indications/Uses
Production of local or regional anaesth by nerve block, infiltration inj, caudal or other epidural blocks. Treatment or prophylaxis of life-threatening ventricular arrhythmias including those associated w/ MI, general anaesth in patients predisposed to ventricular arrhythmias, digitalis intoxication or following resuscitation from cardiac arrest.
Dosage/Direction for Use
Local anaesth for infiltration & nerve block Individualized dosage. Max: 3 mg/kg. Continuous epidural or caudal anaesth & paracervical block for obstetric analgesia Max dose should not be repeated at <1.5 hr intervals. Adult Max: 200 mg. Childn Max: 3 mg/kg. Spinal anaesth Adult Max: 100 mg. IV Cardiac arrhythmia Adult 50-100 mg inj at rate of approx 25-50 mg/min. 2nd dose may be given after 5 min if initial dose does not produce desired response. Max: 200-300 mg during 1 hr period. Patient in whom arrhythmia tends to recur & who are incapable of receiving oral antiarrhythmic therapy Following a single inj, may infuse at a rate of 1-4 mg/min.
Contraindications
Hypersensitivity to lidocaine HCl monohydrate or amide type anaesth. Patients w/ complete heart block or hypovolaemia. Inflammation or sepsis at proposed inj site & in presence of septicaemia. Patients w/ myasthenia gravis, severe shock or impaired cardiac conduction. Epidural or spinal anaesth in patients w/ uncorrected hypotension; coagulation disorders or receiving anticoagulants; serious diseases of CNS or spinal cord (eg, meningitis, spinal fluid block, cranial/spinal haemorrhage, tumours, poliomyelitis, syphilis, TB or metastatic lesions of spinal cord). Antiarrhythmic use in patients w/ supraventricular arrhythmia; Stokes-Adams syndrome or severe degrees of SA, AV or intraventricular block unless patient has artificial pacemaker. Suppresses ventricular pacemaker activity; may cause ventricular standstill. General CI related to epidural anaesth regardless of local anaesth use.
