Indications/Uses
In combination w/ other antiretrovirals for HIV.
Dosage/Direction for Use
Adult 400/100 mg (two 200/50 mg tab or 5 mL oral soln) bd or 800/200 mg (four 200/50 mg tab or 10 mL oral soln) once daily in patients w/ <3 lopinavir-associated mutations. Should not be administered once daily in combination w/ carbamazepine, phenobarb or phenytoin. Childn 6 mth-12 yr weighing >40 kg 400/100 mg (two 200/50 mg tab or 5 mL oral soln) bd, 15-40 kg 10/2.5 mg/kg bd, 7 to <15 kg 12/3 mg/kg bd. Concomitant therapy w/ efavirenz, nevirapine, amprenavir or nelfinavir Adult 400/100 mg bd w/ no adjustment in antiretroviral-naïve patients. May be increased to 6.5 mL oral soln bd in treatment-experienced patients where decreased susceptibility to lopinavir is clinically suspected. Should not be administered as once-daily regimen in combination w/ efavirenz, nevirapine, amprenavir or nelfinavir. Pregnant patient w/ no documented lopinavir-associated resistance substitutions 400/100 mg tab bd. Concomitant therapy w/ efavirenz, nevirapine or amprenavir Childn 6 mth-12 yr weighing >45 kg Max: 533/133 mg bd; 15-45 kg 11/2.75 mg/kg bd; 7 to <15 kg 13/3.25 mg/kg bd.
Administration
Film-coated tab: May be taken with or without food: Swallow whole, do not chew/crush/break.|Oral soln: Should be taken with food: May be administered via feeding tube (not recommended to be used w/ polyurethane feeding tubes).
Contraindications
Hypersensitivity. Co-administration w/ drugs that are highly dependent on CYP3A for clearance & for which elevated plasma conc are associated w/ serious &/or life-threatening events [eg, alfuzosin HCl; ranolazine; dronedarone; fusidic acid; neratinib, apalutamide; colchicine (patients w/ renal &/or hepatic impairment); astemizole, terfenadine; blonanserin, lurasidone, pimozide; midazolam, triazolam; dihydroergotamine, ergonovine, ergotamine, methylergonovine; cisapride; St. John's wort; elbasvir/grazoprevir; lovastatin, simvastatin; lomitapide; salmeterol; sildenafil (only when used for pulmonary arterial HTN)].
