Hyzaar tab 50/12.5 mg: Losartan K 50 mg, hydrochlorothiazide 12.5 mg. Hyzaar Plus tab 100/12.5 mg: Losartan K 100 mg, hydrochlorothiazide 12.5 mg. Hyzaar Forte tab 100/25 mg: Losartan K 100 mg, hydrochlorothiazide 25 mg

Indications/Uses

HTN for patients in whom combination therapy is appropriate. Reduction in the risk of stroke in hypertensive patients w/ left ventricular hypertrophy.

Dosage/Direction for Use

HTN 1 Hyzaar tab once daily. Max: 2 Hyzaar tab or 1 Hyzaar Forte tab once daily. 1 Hyzaar Plus tab for patients titrated to Cozaar 100 mg who require additional BP control. Reduction in the risk of stroke in hypertensive patient w/ left ventricular hypertrophy Initially losartan 50 mg once daily, may be titrated using a combination of losartan & hydrochlorothiazide 12.5 mg. May be increased to losartan 100 mg & hydrochlorothiazide 12.5 mg once daily if needed. May increase dose to losartan 100 mg & hydrochlorothiazide 25 mg once daily if necessary.

Administration

May be taken with or without food.

Contraindications

Hypersensitivity to losartan K, hydrochlorothiazide or other sulfonamide-derived drugs. Anuria. Concomitant use w/ aliskiren-containing products in patients w/ DM or renal impairment (GFR <60 mL/min/1.73 m²).
