Indications/Uses

Chronic Fe overload due to frequent blood transfusions (≥7 mL/kg/mth of packed RBC) in patients w/ β thalassaemia major aged ≥6 yr; when deferoxamine therapy is CI or inadequate in patients w/ other anaemias, those aged 2-5 yr, those w/ β thalassaemia major w/ iron overload due to infrequent blood transfusions (<7 mL/kg/mth of packed RBC); & non-transfusion-dependent thalassemia syndrome in patients aged ≥10 yr.

Dosage/Direction for Use

Transfusional Fe overload Initially 14 mg/kg daily, may be adjusted in steps of 3.5-7 mg/kg daily every 3-6 mth. Patient receiving packed RBC >14 mL/kg/mth Initially 21 mg/kg daily. Maintenance of body Fe level Patient receiving packed RBC <7 mL/kg/mth Initially 7 mg/kg daily. Non-transfusion-dependent thalassemia syndrome Initially 7 mg/kg daily. Consider increasing dose in increments of 3.5-7 mg/kg every 3-6 mth if patient's LIC is ≥7 mg Fe/g dw, or serum ferritin is consistently >2,000 mcg/L & not showing a downward trend. Max dose: 14 mg/kg. Patient in whom LIC was not assessed & serum ferritin ≤2,000 mcg/L Max: 7 mg/kg daily.

Administration

Should be taken with food. Swallow whole w/ water.

Contraindications

Hypersensitivity. High-risk myelodysplastic syndrome patients & those w/ other hematological & non-hematological malignancies who are not expected to benefit from chelation therapy due to rapid progression of their disease. CrCl <60 mL/min or serum creatinine >2x the age-approproate ULN.
