IM Premedicant Administer 1-2 mL 45 min before induction of anaesth.
IV Adult & childn Inidividualised dosing according to age, body wt, physical status, underlying pathological condition, use of other drugs & type of surgery & anaesth.
Adult Spontaneous respiration Initial: 50-200 mcg; supplemental: 50 mcg.
Assisted ventilation Initial: 300-3,500 mcg; supplemental: 100-200 mcg.
Unpremedicated adult 2 mL may be expected to provide sufficient analgesia for 10-20 min in surgical procedures involving low pain intensity; 10 mL as bolus given analgesia lasting about 1 hr for surgery involving moderately painful procedures; 50 mcg/kg will provide intense analgesia for some 4-6 hr for intensely stimulating surgery.
Ventilated patient Loading dose: 50 mcg/mL as a fast infusion of approx 1 mcg/kg/min for the 1st 10 min followed by an infusion of approx 0.1 mcg/kg/min. Alternatively, loading dose may be given as bolus.
Paed Initially 3-5 mcg/kg followed as required by 1 mcg/kg. May be increased up to a max of 15 mcg/kg, followed as required by 1-3 mcg/kg if breathing is artificially assisted.
