Daptomycin Anfarm

Adult & paed (1-17 yr) patients w/ complicated skin & skin structure infections (cSSSI) caused by susceptible isolates of gram +ve bacteria including Staph aureus (including methicillin-resistant isolates), Strep pyogenes, Strep agalactiae, Strep dysgalactiae subsp. equisimilis & Enterococcus faecalis (vancomycin-susceptible isolates only). Adult patients w/ Staph aureus bloodstream infections (bacteremia), including those w/ right-sided infective endocarditis (SAB/RIE), caused by methicillin-susceptible & -resistant isolates. Ped patients (1 to 17 yr) w/ Staph aureus bloodstream infections (bacteremia) caused by methicillin-susceptible & -resistant isolates.

IV Adult Complicated skin & skin structure infection 4 mg/kg in 0.9% NaCl for inj once every 24 hr for 7-14 days, either by inj over 2-min or by infusion over 30-min. Bacteremia 6 mg/kg in 0.9% NaCl for inj once every 24 hr for 2-6 wk, either by inj over 2-min or by infusion over 30-min. cSSSI Renal impairment (CrCl ≥30 mL/min) 4 mg/kg every 24 hr for 7-14 days, (<30 mL/min) 4 mg/kg every 48 hours for 7-14 days. Childn 12-17 yr 5 mg/kg once every 24 hr infused over 30 min for up to 14 days, 7-11 yr 7 mg/kg once every 24 hr infused over 30 min for up to 14 days, 2-6 yr 9 mg/kg once every 24 hr infused over 60 min for up to 14 days. SAB/RIE Renal impairment (CrCl ≥30 mL/min) 6 mg/kg every 24 hr for 2-6 wk, (<30 mL/min) 6 mg/kg every 48 hours for 2-6 wk. Childn 12-17 yr 7 mg/kg once every 24 hr infused over 30 min for up to 42 days, 7-11 yr 9 mg/kg once every 24 hr infused over 30 min for up to 42 days, 2-6 yr 12 mg/kg once every 24 hr infused over 60 min for up to 42 days.
