Indications/Uses

HTN. Heart failure (NYHA class II-IV) in patients receiving standard therapy eg, diuretics, digitalis & either ACE inhibitors or β-blockers but not both; slows heart failure progression, improves NYHA functional class, ejection fraction & signs & symptoms of heart failure & improves quality of life. Improves survival following recent MI in clinically stable patients w/ signs, symptoms or radiological evidence of left ventricular failure &/or w/ left ventricular systolic dysfunction.

Dosage/Direction for Use

Adult HTN 80 mg once daily. Patient whose BP is not adequately controlled May be increased to 160 mg once daily. May add a diuretic or dose may be increased further to max 320 mg if additional BP reduction is required. Heart failure Starting dose: 40 mg bd. Titrate to 80-160 mg bd to the highest dose tolerated by the patient. Max daily dose: 320 mg in divided doses. Post-MI Initially 20 mg bd as early as 12 hr after MI. Titrate to 40 mg, 80 mg & 160 mg bd over the next few wk.

Administration

May be taken with or without food: Administer w/ water.

Contraindications

Hypersensitivity. Concomitant use w/ aliskiren in patients w/ type 2 diabetes; w/ aliskiren-containing products in patients w/ renal impairment (GFR <60 mL/min/1.73m²). Severe hepatic impairment, cirrhosis, biliary obstruction. Severe renal impairment (CrCl <10 mL/min) & patients undergoing dialysis. Pregnancy.
