Indications and Dosage
Intraocular
Miosis during surgery, Reduction of postoperative rises in intraocular pressure
Adult: As 0.01% solution: Instil 0.5 mL into the anterior chamber before or after securing sutures. Do not administer more than 0.5 mL into the anterior chamber. Refer to specific product guidelines for detailed instructions on dose preparation.

Special Precautions
Patient with asthma, acute heart failure, peptic ulcer, gastrointestinal spasm, hyperthyroidism, urinary tract obstruction, Parkinson's disease. Pregnancy and lactation.
Adverse Reactions
Significant: May increase surgically induced intraocular inflammation.
Eye disorders: Corneal clouding, persistent bullous keratopathy, retinal detachment; postoperative iritis (following cataract extraction).
Gastrointestinal disorders: Epigastric distress, abdominal cramps, vomiting.
Nervous system disorders: Headache.
Renal and urinary disorders: Bladder spasm.
Skin and subcutaneous tissue disorders: Sweating.
Vascular disorders: Flushing.

Patient Counseling Information
This drug may cause temporary blurring of vision, if affected, do not drive or operate machinery.
Overdosage
Symptoms: Headache, vomiting, abdominal cramps, salivation, diarrhoea, asthma, syncope, bradycardia, and hypotension. Management: Supportive treatment. For severe systemic toxicity, anticholinergics may be given.
Action
Description:
Mechanism of Action: Carbachol, a choline ester, is a direct-acting parasympathomimetic agent. It stimulates the muscarinic receptors in the eye, causing constriction of the iris and ciliary body, leading to a reduction in intraocular pressure.
Onset: Miosis: Within 2-5 minutes.
Duration: Miosis: Approx 24 hours.
