Azithromycin dihydrate
Indications/Uses
Acute bacterial sinusitis; acute bacterial otitis media; pharyngitis/tonsillitis; acute exacerbation of chronic bronchitis; mild to moderately severe community-acquired pneumonia; skin & soft tissue infections; uncomplicated Chlamydia trachomatis urethritis & cervicitis.
Dosage/Direction for Use
Adult 500 mg/day for 3 consecutive days. Uncomplicated Chlamydia trachomatis urethritis & cervicitis 1,000 mg as single dose.
Administration
May be taken with or without food: May be taken w/ meals to reduce GI discomfort.
Contraindications
Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotics.
Special Precautions
Reports of serious allergic reactions including angioneurotic oedema & anaphylaxis, acute generalised exanthematous pustulosis & DRESS; Clostridium difficile-associated diarrhea; exacerbations of myasthenia gravis symptoms & new onset of myasthenia syndrome. Risk of QT interval prolongation. Stop treatment if liver dysfunction has emerged. Observe for signs of superinfection. No efficacy data on prevention of acute rheumatic fever. Do not co-administer w/ ergot derivatives. Contains sulfur dioxide. Patients w/ rare hereditary problems of galactose intolerance, total lactase deficiency or glucose-galactose malabsorption should not take this medicine. Patients w/ significant hepatic disease; severe renal impairment (GFR <10 mL/min). Pregnancy & lactation.
Adverse Reactions
Diarrhoea, abdominal pain, nausea, flatulence. Anorexia; dizziness, headache, paraesthesia, dysgeusia; visual impairment; deafness; vomiting, dyspepsia; rash, pruritus; arthralgia; fatigue; decreased lymphocyte count, increased eosinophil count, decreased blood bicarbonate.
Drug Interactions
Reduced peak serum conc w/ antacids. Increased serum levels of P-gp substrates eg, digoxin & colchicine. Increased conc of phosphorylated zidovudine. Theoretical possibility of ergotism w/ ergot derivatives. Post-marketing reports of potentiated anticoagulation subsequent to co-administration of azithromycin & coumarin-type oral anticoagulants. Elevated Cmax & AUC0-5 of ciclosporin. Increased conc w/ nelfinavir.
